L4-L5 Disc Bulge: Treatment, Exercises & Recovery Time

If you've been diagnosed with an L4-L5 disc bulge, you're probably worried about what happens next. Will you need surgery? How long will it take to recover? Will you be in this much pain forever?

Here's the good news: 85-90% of people with disc herniation symptoms get better within 6-12 weeks without surgery 1Source: National Library of Medicine.

What is a lumbar disc bulge (L4-L5)?

Your spine has cushioning discs between each bone. The L4-L5 disc sits between the fourth and fifth lumbar vertebrae in your lower back.

When working as they should, these discs cushion your movements and support your upper body weight. A disc bulge happens when the disc pushes out beyond its normal position, which can press on nearby nerves.

The L4-L5 area is especially prone to injury because it carries the most weight in your lower back and does most of your bending and twisting. This is why the majority of lower back disc problems happen at this level.

Disc problems come in different levels of severity:

  • A bulged disc means the outer disc wall stays intact but pushes outward.
  • A herniated disc (also called a slipped disc or prolapsed disc) means the inner gel-like material breaks through the outer wall.
  • A sequestration means the disc material breaks off completely and floats freely.

Surprisingly, larger herniations often heal better than smaller bulges. Sequestrated discs heal 88% of the time compared to just 13% for bulges, because larger herniations trigger a stronger healing response in your body. 2Source: PubMed

Common symptoms

An L4-L5 disc bulge usually affects the L5 nerve. This causes pain that can start in your buttock and usually travels down the outer side of your thigh and calf. The pain often reaches the top of your foot and big toe. You may also feel numbness and tingling in these same areas, particularly between your big toe and second toe.

Weakness is another common sign. You might find it hard to lift your leg or foot upward or extend your big toe. In worse cases, you might develop foot drop, where your foot slaps the ground when you walk.

Many people find their leg pain is more annoying or concerning than their back pain. This might seem strange, but it's quite common when a nerve is compressed.

Less commonly, if the bulge is more to the side, it can affect the L4 nerve instead. This causes pain in the front of your thigh, quad muscle weakness, and a weaker knee reflex.

Remember: how severe your symptoms feel doesn't always match the size of your bulge on scans. Small herniations can cause intense pain if the nerve is irritated, while large herniations might cause surprisingly few symptoms.

Treatment options that work

The research is clear: non-surgical treatment works very well for L4-L5 disc bulges. While surgery gives faster pain relief at first, results after 2 years show no real difference between surgery and non-surgical treatment. 3Source: PubMed

Your body's natural healing is powerful. It will naturally break down and absorb the bulging disc material within about six months. The larger the herniation, the better it tends to heal – that's nature's way of fixing the problem.

At Rasura Chiropractic Centres, we use several proven methods to treat disc bulges. One of the approaches that can be used is called flexion-distraction technique (also known as Cox Technique).

This gentle, rhythmic treatment increases the space between your spine bones and creates negative pressure in the disc. This helps pull the bulge away from compressed nerves. Research shows that 90% of patients with confirmed disc herniation on MRI scans improve within three months with spinal manipulation such as the Cox Technique. 4Source: PubMed

We also use targeted exercises, which show excellent results. For patients who need extra help, dry needling can be very effective as well. This technique releases muscle tension, improves blood flow, and reduces inflammation around compressed nerves. Dr Lillie Lines, who practices at our Alexandra Headland clinic, does this for her patients and has a high success rate of helping them out of pain.

The key to recovering without surgery is staying active rather than resting for extended periods. Staying in bed for more than 1-2 days isn't recommended and might actually slow your recovery by weakening your muscles and making you stiffer.

Helpful exercises for L4-L5 bulges

Cat-cow exercise

This simple yet effective stretch helps your spine move better. When you do it along with other gentle stretches and exercises, it can help reduce pressure on your disc and improve how you move.

Get on your hands and knees with your hands under your shoulders and knees under your hips. For the "cow" part, slowly arch your back down, lifting your chest and tailbone up whilst keeping your neck relaxed. Then move to the "cat" part by rounding your back up, tucking your chin to your chest and pulling your belly button in.

Move slowly between these two positions for 10-15 times. Breathe in during cow, breathe out during cat.

If your back or leg pain gets worse or moves further down your leg, stop and talk to your practitioner.

Core strengthening

Core strengthening exercises strengthen the muscles around your spine, reducing pressure on the disc and improving stability.

Try these exercises when you can actively activate your core without pain :

Bird dog

On hands and knees, extend opposite arm and leg while keeping your hips level and core tight, then alternate sides.

Dead bug

Lie on your back with arms extended upward and knees bent. Lower opposite arm and leg simultaneously while keeping your lower back flat, then alternate sides.

Planks

Support your body on forearms and toes with core engaged and body straight. Hold front plank for 40 seconds, then side plank for 40 seconds on each side.

Other exercise tips

If your symptoms have lasted more than 3 months, gentle nerve gliding exercises can help. These special movements help your sciatic nerve slide more freely and get better blood flow. Your chiropractor will be happy to take you through these steps.

What you avoid is just as important as what you do. During the early healing stages, avoid repeated forward bending, especially while lifting. This means no sit-ups, crunches, toe touches, or heavy squats and deadlifts at first. Sitting for long periods significantly increases disc pressure compared to standing, so take regular movement breaks every 30-45 minutes.

Recovery timeline: what to expect

During the first 1-2 weeks, you'll likely have the worst pain, heat and inflammation. This is when the disc material is most irritated and your body's healing response is strongest. It's uncomfortable, but this inflammation is actually part of healing.

From 2-12 weeks, symptoms gradually improve as your disc starts to heal and scar tissue forms. Most people notice real improvement during this time, with less leg pain and better sitting tolerance by around 4 weeks.

From 6-12 weeks onwards, you'll see significant functional improvement, though you might have occasional flare-ups. Full recovery typically happens within 8-12 weeks for most patients, though the disc structure continues changing for several months.

A few things can affect how fast you heal. Things that help you recover faster include: having pain for less than 6 months, a larger bulge, being younger, good mental health, and sticking to your exercise program.

Aspects that might slow recovery include diabetes (which slows nerve healing), severe muscle weakness at the start, smoking, inactive lifestyle, and having symptoms for more than 6 months.

Most people can return to light daily activities like walking within days, find sitting easier by 3-4 weeks, and return to bending and lifting with proper technique by 8-12 weeks.

When to get urgent medical help

While most L4-L5 disc bulges heal well without surgery, certain warning signs need immediate medical attention.

Cauda equina syndrome, a rare but serious condition where the nerve bundle at the base of the spine becomes severely compressed, is a surgical emergency that needs treatment within 24-48 hours to prevent permanent nerve damage. 5Source: AANS

Get emergency care immediately if you have saddle numbness (numbness in your groin, buttocks, or inner thighs), can't start urination or have a weak stream, bowel problems, weakness in both legs, or sudden sexual problems along with your back pain.

You should also see a specialist promptly if you develop progressive muscle weakness, especially severe weakness where you can't lift against gravity, new problems during treatment, or foot drop that isn't improving.

Also, if symptoms don't respond to treatment after 6-12 weeks, you need a specialist referral.

Your path to recovery

When you combine the right treatments – learning about your condition, doing specific exercises, getting hands-on care, and making lifestyle adjustments – most people recover fully and get back to their normal routine within 3-6 months without surgery.
